CONSENT TO RESEARCH TREATMENT AND RELEASE OF LIABILITY
This Consent to Treatment and Release of Liability (“Agreement”) is entered into by and between the subject and/or patient (“Subject”) and DripDok. Please read the following terms carefully. By selecting “I Agree”, Patient acknowledges that they have read, understood, and agree to be bound by the following terms and conditions.
- Consent to Treatment/Research Study:
- The subject (patient) hereby consents to receive research treatment, care, and services provided by DripDok, its employees, agents, and representatives. The subject understands that the healthcare provider will explain the nature and purpose of the treatment, procedures, and other services, and Subject has the right to agree or refuse the proposed treatment and services.
- Assumption of Risk:
- Patient acknowledges that all medical treatment and services have the potential for risks and benefits, and that the outcomes are not guaranteed. Subject voluntarily assumes all risks associated (known and unknown) with the treatment and services provided by DripDok.
- Release of Liability:
- The Subject, for themselves and their heirs, executors, administrators, and assigns, hereby release and forever discharge DripDok, its employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action, arising out of or related to any loss, damage, injury, or death, that may be sustained by Patient as a result of receiving treatment and services from DripDok.
- Governing Law:
- This Agreement shall be governed by and construed in accordance with the laws of the country of Singapore and in which the subject receives the research treatment and services.
- Entire Agreement:
- This Agreement contains the entire agreement between the parties and supersedes all prior oral or written agreements, understandings, or representations.
By selecting “I Agree” on any form, The Subject or Patient acknowledges and agrees to the terms and conditions of this Agreement without any doubt or reservation or coercion.
